Orthodontia coverage is included in the plan for children under age 19!
Coverage for the Preventive Services benefits start the day the policy is effective!
Dental expenses for Preventive & Basic Repairs are covered at 80% of the R&C charges!
- Fluoride treatment is provided for children to age 16.
- Coverage can be on an adult, parent & child(ren) or the entire family.
- Coverage is available to applicants age 18 and over.
- Your correct premium area is determined by the first 3 digits of your ZIP CODE - see monthly premium page. (For example, in California - if your Zip code starts with 930 thru 934 use Area 6 rates.)
- Premium payments - your choice of either monthly by automatic bank withdrawal OR bill direct quarterly.
- First premium and one time policy fee must accompany the applcation.
